The lore

In 2001, NASA researchers Arnauld Nicogossian, D.F. Pober, and S.A. Roy published "Evolution of Telemedicine in the Space Program and Earth Applications" in Telemedicine Journal and e-Health — a formal survey of what remote medical diagnosis looked like aboard NASA missions and where it was headed. The International Space Station, they noted, was designed to serve as a testbed for telemedicine technologies to enable future deep-space missions. Crew health, remotely monitored. Diagnostics, autonomous. The waiting room, eliminated by design constraint.

That paper described infrastructure already operational in orbit in 2001. It described the gap between what those systems could do and what was available on the ground, and it projected forward to missions lasting months or years, millions of miles from any hospital.

The med bay of the future is not a concept. It has a publication history, a hardware manifest, and a program office. What exists aboard the ISS is a reduced-gravity version of the problem. The version designed for the rest of us is a different engineering brief entirely — more capable, less constrained by launch weight, built for a patient population that does not have a return window.
